Commitment Fee: $100

Commitment Fee will go toward your total payment.

Commitment Fee will be forfeited if appointment is not canceled within 36 hours of the time of your appointment.

Remaining balance will be due at your appointment.

If you arrive at your appointment with previous PMU that you have not made me aware of and it is something that I’m not comfortable covering because it has not faded enough, your commitment fee will be forfeited and your appointment will be canceled.
